Rule 4.1. Filing of Claim of Appeal

(A) Any person aggrieved by the entry of a decree by a trial judge may appeal to the Appellate Division of the court by filing with the court a claim of appeal on a form promulgated by the court. The claim of appeal shall include a request for a transcript of the testimony and decision in the case, or any part thereof required for purposes of the appeal.

(B) The party filing the claim of appeal shall forthwith forward a conformed copy of said claim of appeal to all other parties which shall include all dates, amounts, and times contained in the original claim of appeal filed with the court.
